Registration warns staff when a patient may already exist, so one person keeps one record. The ID photo follows the patient to every queue, the cashier screen, released results and APE/PEME reports.
When a result is released, CDMS Labs stores the full printed document with a SHA-256 hash. A reprint years later matches what the patient first received, even after templates, letterhead or patient details change. Every view of a patient's history is audited, and doctors see only their own patients.
